Ontario Government Triples Support for Older Adults in Northumberland County

Ontario Trillium Foundation’s $456,600 investment will strengthen local care and expand access to proven community-based program.

COBOURG, ON – On Friday, a representative from the office of MPP David Piccini joined
Community Care Northumberland (CCN) to recognize a $456,600 Grow grant from
the provincial government’s Ontario Trillium Foundation (OTF).
This three-year investment will support the expansion of CCN’s WrapAround for Older Adults
program, transitioning it from a volunteer-led model a staff-led approach, and tripling its capacity
to support older adults across Northumberland County.


“This is an incredible investment for our community,” said David Piccini, MPP, Northumberland
Peterborough-South. “Our government is proud to support the work of Community Care
Northumberland and the expansion of their WrapAround Program. By investing in this program,
we are connecting 300 individuals with complex needs to the care they need, close to home.”
The WrapAround for Older Adults program is a person-centred, team-based planning process
that develops individualized support plans for older adults with complex needs. Over the past
four years, the program has supported 82 participants, with 34 successfully transitioned out
following positive outcomes. It is currently supporting 46 active participants.


With support from the Ontario Trillium Foundation, the program will transition to a staff-led
model to improve consistency, capacity, and continuity of care. Trained staff will each support
30–35 participants, while volunteers will continue in valued Wraparound Support Mentor roles.
The expanded model will enable the program to support more than 100 older adults annually —
reaching approximately 300 participants over the three-year funding period — while addressing
key needs including housing, transportation, healthcare access, and social connection.

greater consistency and safety, and ensure that no one has to navigate complex challenges

Community Care Northumberland (CCN) is a non-profit charitable organization providing
community support services to residents of Northumberland County. CCN’s programs support
older adults, caregivers, and individuals with complex needs to live safely and independently at
home. The WrapAround for Older Adults program is the only program of its kind in
Northumberland County, coordinating formal services and natural community supports into a
